Haitis Wirtschaft hofft auf mehr Touristen. Das Land, das eines der ärmsten der Welt ist, verzeichnet trotz der Erdbebenkatastrophe immer mehr Hotels und Resorts.

Coxalgie - Hüftgelenksschmerzen... 

Das Wort „Coxalgie“ beschreibt Hüftgelenksschmerzen.Im Hüftgelenk bewegen sich Rumpf/ Becken und Oberschenkel gegeneinander. Das Hüftgelenk ist ein sog. Napfgelenk, zuständig für Beuge-, Streck- und Kreiselbewegungen, und natürlich auch für die Bewegungen, die seitwärts von der Körperachse wegführen - die sog. Abduktion - und natürlich das Heranführen an die Körperachse - genannt Adduktion. 

Die Gelenkkapsel ist durch kräftige Bänder verstärkt.Die Coxalgie kann viele Ursachen haben.Die häufigste ist jedoch die Arthrose (  Informationen: Arthrose ) , eine durch Abnutzung bedingte, also degenerative Gelenkerkrankung. Sie tritt überwiegend erst im Alter auf. Bei der Coxarthrosis deformans (Arthropathia deformans) bestehen schmerzhafte Gelenkveränderungen, die zunehmend die Funktion des Gelenkes aufgrund eines Mißverhältnisses zwischen Belastung und Belastbarkeit stören. 

Betroffen ist primär das Hüft- und Kniegelenk. Die Entstehung einer Arthrose und einer daraus folgenden Coxalgie kann durch mehrere Faktoren begünstigt werden. Z. B durch: 

• Vorschädigung infolge eines Unfalls

• eigenständige Gelenksentzündung

• permanente Fehlbelastungen, z. B Fehlstellung der Beine, genannt X- oder O-BeineUm einer Arthrose vorzubeugen, ist es ratsam, diese Fehlbildungen zu erkennen und zu beseitigen. 

Eher selten können auch Tumore die Ursache sein. Meistens gehen diese von der Innenhaut der Gelenkkapsel (Membrana synovialis) oder von der Gelenkkapsel selbst aus. 

Gutartige Tumore sind: 

• Fibrome (Bindegewebsgeschwülste) 

• Lipome (Fettgewebsgeschwülste) 

• Hämangiome (Wucherungen von Blutgefäßen) 

Ein bösartiger Tumor ist: 

• malignes Synovialom 

Hüftgelenksschmerzen entstehen auch bei einer Gelenkentzündung (Eitrige Gelenkentzündung) infolge einer Systemerkrankung, meistens sind dann aber mehrere Gelenke betroffen. Als Ursache gelten hauptsächlich rheumatische bzw. rheumatoide (rheumaähnliche) Erkrankungen. Bei Jugendlichen kann eine Coxalgie auch im Rahmen einer systemischen juvenilen chronischen Arthritis (Still Syndrom) auftauchen. 

Das Still Syndrom ist eine meist symmetrisch verteilte Polyarthritis (Entzündung in mehreren Gelenken). Begleitende Symptome sind Fieber, geschwollene Lymphknoten, sowie eine Milz- und Lebervergrößerung. Des Weiteren können Hüftgelenkschmerzen auch Folge einer Infektionskrankheit sein, z. B. nach Masern, Mumps, Röteln oder Windpocken. Diese werden dann auch als sog. parainfektiösen reaktiven Arthritiden (Gelenkentzündungen) bezeichnet. Therapie: Coxalgie Hüftgelenksschmerzen Die Therapie einer Coxalgie bzw. von Hüftgelenkschmerzen richtet sich einerseits nach Ursache (kausale Therapie) und andererseits nach dem Symptom, also dem Schmerz (symptomatische Therapie).

Je nach Ursache wird eine entsprechende Therapie gewählt, z. B. wird man bei bestehenden parainfektiösen reaktiven Arthritiden diese entsprechend medikamentös behandeln. Ein wichtiges Thema im Zusammenhang mit der Coxalgie ist eine ursächliche Arthrose. Häufig wird hier zu einer Operation geraten (Einsatz eines künstlichen Hüftgelenks,…). Hier muß allerdings klar gesagt werden, daß dieser Schritt gut überlegt sein sollte, denn oft wird er einfach auch viel zu früh empfohlen. Die spezielle Schmerztherapie z. B. verfügt bei Hüftgelenkschmerzen mit diesem Hintergrund über sehr gute Möglichkeiten bestehende Leiden zu lindern. 

Häufig kann so eine Operation vermieden bzw. sehr lang hinausgeschoben werden. Ansonsten gilt bei chronischen Hüftgelenkschmerzen bzw. einer Coxalgie generell, daß dies, wie bei jeder anderer chronischer Schmerzkrankheit auch, zu einer Art Überreizung der Schmerznerven führt. Dies führt zu Dauerschmerzen, was wiederum eine Überreizung beinhaltet, und ein Teufelskreis beginnt. Spätestens hier sind (ergänzend) gezielte schmerztherapeutische Behandlungsmethoden gefragt. Diese sind oft bei verschiedenen Grundkrankheiten die gleichen, da sie sich nach dem Schmerz und nicht mehr nach seiner Ursache richten. 

Chronische Hüftgelenksschmerzen erfordern dabei praktisch immer eine Kombination von verschiedenen Therapieverfahren und eine optimale Hilfe kann im Prinzip nur durch tägliche Therapien im Rahmen einer stationären Behandlung in einer Schmerzklinik erreicht werden. 1. Möglichkeiten die Schmerzen symptomatisch (auch ambulant) zu behandeln: - Medikamentöse Schmerztherapie. Die Mittel erster Wahl sind peripher wirkende Analgetika (Schmerzmittel, die am Ort der Schmerzentstehung wirken), spez. sog. nicht steroidale Antirheumatika (Rheumamittel), hierbei möglichst lang wirkende und magenschonende, z. B. Mobec®. 

Bei starken, schmerzhaften Muskelverspannungen können ergänzend Muskelrelaxanzien (Mittel zur Muskelentspannung) (z. B. Norflex®, Mydocalm®) eine Lösung sein. Schwere Schmerzen sind manchmal dagegen nur durch zentral (im Gehirn / Rückenmark) wirkenden Analgetika (z. B. Tramadol, Valoron N®) zu lindern. Auf die Gefahr einer Schmerzmittelgewöhnung oder gar -abhängigkeit ist grundsätzlich zu achten. Eine Kombination mit schmerzdistanzierenden Antidepressiva (Mittel gegen Depression, u.a. aber auch bei Coxalgie wirksam) kann in vielen Fällen Schmerzmittel einsparen. - Physikalische Therapie (Krankengymnastik, T.E.N.S. …)- Akupunktur.
